Hi,
I have made a simple model of a 2D moment frame. when running adaptive pushover analysis, it fails to apply loads, while i made a lot of attempt to avoid any mistake in modeling. it seems that the adaptive pushover module doesnt works well (at least for some cases), can you help me to solve this?

Indeed, there is a bug in v6.5, when employing adaptive response control. The bug will be fixed in the upcoming v7 that will be published in spring of 2014.
Regarding your model, it wll run without problems if you change to adaptive load control scheme.
