Indeed, the element behavior varies significantly with the length of the plastic hinge (and often without physical meaning).
It is strongly recommended to leave the default value.

In some of the employed methods (e.g. Artificial Accelerogram Generation) the envelope shape is input by the user. In others (e.g. Synthetic Accelerogram Generation & Adjustment) the envelope shape is provided by the algorithm. You need to check the help system of the program and the methodology...

It depends. If the wall is part of the load resisting mechanism, it should be modelled as a frame element. The cover of the section is the cover of the jacket. Reinforced concrete jackets always have concrete cover, which is the concrete outside the jacket reinforcement (typically 2-3 cm as is the cover in plain sections).
